POLICY DISCLOSURES CLIENT INFORMATION Client information will be securely stored, on-site, for at least 3 years; per the MDH requirement. A new consent/health disclosure form must be completed anytime a client comes to the tattoo studio for a new appointment/session. A valid photo ID is required by the MDH to accompany the paperwork. PROCEDURE SITE/ BODY ART DESCRIPTION Artists/Technicians reserve the right to refuse and/or cease services for any reason at any time they see fit. Payment for services rendered is still due regardless of level of completion. Procedure site must be free from any blemishes, open/healing wounds, sunburn, scratches, implanted medical devices (unless ok’d by physician), or any other physical conditions that would negatively impact the quality of the tattoo, or the outcome of the overall procedure. Earth Child Tattoo LLC will not tattoo genitals, with the exception of reconstructive/therapeutic/self-esteem tattoos – such as mastectomy scar cover ups, nipple reconstruction, etc. said services will be offered under particular circumstances and only during private appointments, with as much modesty and respect as possible, in order to provide optimal comfort to our clients. Earth Child Tattoo LLC will not tattoo any gang affiliated symbols, font, or representations, etc. Nor will we tattoo any hate speech, racist, sexist, or otherwise derogatory type of tattoos. DEPOSITS We do require a $100 non-refundable deposit to be paid upon booking of all appointments. This deposit ensures that your artist will be holding that date/time for your appointment. Under no circumstances will this deposit be refunded to you. Deposits do act like a credit towards the cost of your tattoo, as long as you keep your appointment date, time, and booked design. No-shows, last minute/short notice cancellations, last minute design changes yielding a reschedule etc. are some of the circumstances that may result in the forfeiture of a deposit. FREE TOUCH-UP We always stand by our work. That being said, we are human, and so is your skin. Things happen. That is why we offer one (1) complimentary touch up, if it is needed. This is offered under the following criteria; - Must notify your artist within 60 days of your final tattoo appointment, letting us know you wish to have a touch up. We will likely ask for a photo to see how it is progressing so we can gauge the amount of time it will take. Regular aging does not qualify for a free touch up. - Certain areas on the body do not qualify such as; fingers, hands, feet, elbows, knees, and other body areas where you are warned in advance that fall-out is likely and touch ups are at client expense. - You should be following aftercare as instructed, unless you had some sort of negative reaction to either the bandage or ointment. Client neglect during aftercare does not warrant a free touch up. - The purpose of the free touch up is for things like fine line work that may have lost some ink, or designs with heavy saturation that healed unevenly in a small for some reason, or perhaps a small scratch occurred during the healing process... things of this nature. ZERO TOLERANCE We have a strict zero tolerance policy for sexual harassment in our studio. We are all adults here, and we can certainly take a joke. That being said, this is our work place. We are here to create art. In order to do so to the best of our abilities, that does require a certain type of environment. Earth Child Tattoo LLC takes the safety and well-being of our staff extremely seriously. We have worked very hard to build this space where our clients come to place their trust in us, and feel safe, calm, and at ease while we turn their ideas into a permanent reality for them. In order for our clients to feel that way, we need to as well. Any instances of sexual harassment will result in the ceasing of any services rendered, payment will be due for said services, and the offending party will be removed immediately. PHOTOGRAPHS We have no problem if you want to document your time here with us. Please do! That being said, we ask that you please be mindful and respectful of the other clients in the studio & be sure to ask their permission as well if they happen to be in any of your photos.
